Alternate Transportation Request Form.

COVID Protocols for Returning to Play (Positive Case)
: Any student-athlete or coach returning from a positive COVID case will need to provide a doctor’s note to the school nurse indicating that they are free of symptoms and allowed to return. The nurse will ensure that the athletic trainer and coach(es) are notified. The athletic trainer will then institute the state protocols for return to play with the student-athlete.
